Social Worker – Physical Rehabilitation Hospital (Newlands)
CRITICAL OUTPUTS
- Assess patients’ psychosocial needs and provide emotional support.
- Advocate for patient rights and coordinate resources for optimal recovery.
- Act as a liaison between patients, families, and healthcare teams to ensure clear communication.
- Develop individualized discharge plans, including home care and community resource coordination.
- Provide counselling and education on coping strategies, healthcare options, and lifestyle adjustments.
- Work collaboratively with interdisciplinary teams to implement holistic care plans.
- Address urgent psychosocial concerns such as financial hardship, housing insecurity, and mental health crises.
- Maintain accurate patient records and ensure compliance with healthcare regulations.
REQUIREMENTS
- Education: Bachelor of Social Work (BSW) degree & SACSSP registration.
- Experience: 3–5 years as a Social Worker, preferably in healthcare or physical rehabilitation.
Other:
- Code 8 driver’s license and own reliable transport.
- Ability to work in a pressurized environment with on-call and overtime requirements.
- Strong interpersonal, communication, and ethical decision-making skills.
- Experience in interdisciplinary collaboration.
