United started sluggishly, controlling the majority of possession but failing to create clear-cut chances.
Plzen’s compact defensive setup left little room for United to exploit, and the first half ended goalless.
After halftime, United aimed to inject more energy into the game, but it was Plzen who struck first.
A sloppy pass from Andre Onana to Matthijs de Ligt allowed Plzen to press effectively and win the ball high up the pitch.
Matej Vydra capitalised on the mistake, slotting home to give the home side a shock lead.
Facing the prospect of another disappointing result, manager Ruben Amorim made three changes, including the introduction of Rasmus Hojlund.
The Dane made an impact, equalising after a brilliant solo run from Amad Diallo down the right flank.
With time running out, United searched desperately for a winner.
Their breakthrough came in the dying minutes when Bruno Fernandes played a clever free kick to Hojlund, who rolled his marker and fired into the bottom corner.
Despite a poor performance for much of the game, Amorim will be relieved to secure the three points.
However, he’ll know improvements are needed moving forward.
